I don't really think "cool" will be the applicable to a situation
where you click and then have to select something from a list, instead
of immediate action :)
  

Of course, you should've read "useful" instead of "cool" ;-). In Unigraphics for example, you get the immediate action that you like, but if you hover above an item and wait for about half a second before clicking, you'll get a pop-up menu. This delay is user configurable and after that delay the pointer changes slightly as a hint. I like it this way, but maybe that's because I'm  used to it.

Note that I'm not claiming that Unigraphics has a great UI though, it even had an "edit" menu at the left of the "file" menu :-(

But if anyone wants to implement that, let's discuss the details. Of
course it should be optional, for most people and drawings click and
alt-click works just fine.
