My thought was to use "active/inactive", then I read that you wrote that ;)
You might also use "applied", but I think "active" more clear.
As for the icon, maybe even something as simple as a green dot and red or grey dot would work nicely. They already exist in common icon sets, even, to demark the same idea.

Maybe a set of 2 gears (with a cross through them if deactivated).
