The spiral gradient renderer would have to remain non-cairo, I think that's inevitable. But the patch includes the renderer, and switch to cairo does not have to be all-or-nothing anyway - for example filters would still have to be rendered outside of cairo anyway.
That reminds me of Carl's talk at LGM last year where support for SVG filters came up as something that would love to see done. Another thing is that xlib seems to support conical gradient fills now, and doesn't Cairo rely on xlib?
