With Inkscape build 031231 the preferences file is picked up and the preferences are used. With Inkscape build 040106 the preferences file is not picked up.
Swapping preference files does not alter things. I read this as a change in Inkscape and nothing to do with the preferences file.
Can someone help here? For example, Bob, what happened between these two releases?
I can confirm that on Linux it still works as it should.
Mental, perhaps some of your fixes should be changed or reverted in the WIN32 branch?

Well, there's not much to change or revert -- it's really the identical code between Linux and Win32 now.
Ah, there is one significant change -- on Win32, the preferences directory name has changed to "Inkscape" rather than ".inkscape" to conform more to Win32 conventions for these things.
