Maybe a warning dialog or a status bar update would be positive to notify the user if a comment is orphaned in someway. I know though this would require some intelligent coding.

Obviously we wouldn't want to add an advertisment comment to the document if one already existed, or they'd start piling up every time one was saved.

Well, if we had code that could determine that a comment had been orphaned, the same code should be able to move the comment to an appropriate place automatically. I just don't think it's doable though.
Ah, btw, for those of you not on the Sodipodi list, I've been talking with Lauris about this and he raised some good points that have sort of changed my mind... so maybe a basic comment-preserving implementation is worth it after all, even though we can't automatically deal with orphaned comments.
Since we have the XML editor, it's not like the comments are getting lost totally behind the user's back -- and an XML file with a lot of comments is probably either going to be only edited trivially, or partly edited in the XML editor anyway (in which case the user can see and fix orphans).
