I think that this might be related to that tool-switching bug that someone noticed last week, but I can't be sure. I'm pretty certain, though, that this is not cairo-pdf related.

You don't have any gtk stuff in your PATH, do you? If so, look at the inkscape.bat file that I added to the directory a couple of weeks ago. What it does is run Inkscape with a minimal path. Edit it to have whatever directory you have inkscape installed to and try double-clicking it.
