Yeah, This is the problem I was looking into Saturdy Evening. I'll be looking at it again on Tuesday or Wednesday. If anyone else wants to have a go at it, It seems to be around sp_document_undo or sp_document_maybe_done. It seems that if you undo when there is nothing to undo, the undo list doesn't get updated when you create new object.

Problem is kdbg was crashing on me when I tried to set a breakpoint inside sp_document_maybe_done to see what was happening there.
