It would help if Inkscape could produce a pdf file in the PDF X-3 format. That gets around the CMYK problem insofar as many printers have new printer and prepress software (e.g., LSI, Brandt-Doubleday) that will handle the conversion automatically from RGB to CMYK.
I can always import into Scribus and then export in PDF X-3 but that is an extra step.

Sorry about getting low-level on you :), but this basically means that someone needs to patch Cairo to support X-3 output.

Scribus will spit out X-3. If you happen to know, what does that program use instead of Cairo? Or is it an add-on?

Scribus uses Cairo just for on-screen rendering. For exporting PDF it uses its own library.
