The line style for "CAD" is a 1px line with markers. you only can
customize by the tool the distance to original by the offset parameter
and the size of the text. Maybe can add to the todo list use the last
line type and markers used in the pen/pencil tool, ¿are you sure is a
good idea?

I think it would offer the most flexibility with the least amount of work and offer a way for people to choose marker types, though truly it's not that much work to copy and paste a line style to all the markers at once, so... it's up to you. A 1px line at 300 dpi would be nearly invisible in print, though so... there are advantages to handling it either way. However the problem arises, what if the line is white, or transparent, or the width is 0? Then it will look like nothing is happening, I assume. Maybe it's better to leave as is for now.

Any way the best approach I think is:
On convert to CAD line the generated line become selected, sou you can
apply a clipboard style with a click. And you can use your own lines
and markers to it

Big markers -by the marker or by the stroke width- go outside the limit
anyway.

This is true, though the line is printed as a reference only, so it doesn't make much of a difference. You would not set them very thick, but just thick enough to see on the printed product for reference.

I can see there are many potential problems with using the last line width/colour/endpoints. Maybe you can think of a way to do it best, but honestly, I love where the ruler is going, even as you have it currently. :)
