It is not clear how this example was produced. - which version of Inkscape? current trunk shows four options under "Insert new nodes into selected segments". The options are: min x, max x, min y, max y. Does it matter which option is used?

I think the main question will be whether the relevant code is found in Inkscape trunk or whether it is part of lib2geom. Fixing it in Inkscape trunk is relatively easy, and has been done before, for example: https://bugs.launchpad.net/inkscape/+bug/332735
