
Please, I need further information:
In step 3, I don't see a save as "Cairo PDF" only PDF. Where is Cairo? How do you compare the two?
In File/Server Properties tab, I created the banner size I'm attempting to create. When I try to print, that "fancy" sized paper does not come up as a selection in the printer set-up box (should I be able to select that one particular paper size? Only the paper sizes from HP are available. How do I select that one size paper?)
Step 5: I believe I checked the document properties that the page size was correct but only 11 inches of the document showed in the Acrobat Reader box.
I am so incouraged by your last line, "I am able to print huge/or custom ....." Very cool.

I've been checking out some of the commercial page lay-out & publishing apps available. For the classroom, Print Master (I have an old version,12) has a very easy to use and print banner component. It sets the banner length as you type so, you start out with one page and as you type the banner lengthens. This is very cool but is rather limited in the document design area. I'm thinking about checking out some of the Serif products. There support staff assures me that there are no problems printing banners to different lengths. And, the applications offer my students powerful (as far as I can tell)design components so that they can draw, etc. as much as they want. I include these thoughts in the hopes that Inkscape incorporates some or all of these features.

I am able to print huge and/or custom works this way without problems in many different printers.
Wild wild guess: if everything else fails, try printing an exported bitmap with a resolution of 600 dpi or less.
