Well I'm not saying it can't be used for commercial purposes just that I would like to know. but if a CC works better for the purpose of having on the site, I have no problem changing it. Then I would prefer a http://creativecommons.org/licenses/by-sa/4.0/ 
Especially if its going to be used on the website, which is flattering that you would even consider using it :). 

I can add the badge and update the files if that works, though some input from other on the list would be cool too 

I would include my full name too :)

I would like to know if someone does decide to make money off this little
booklet, to let me know out of common courtesy or decency.

so its PD but a reasonable “hey we printed this out and we made a few bucks
out of it, thanks Judah!” would be great or they could just make their own
guide :) it took me less than an hour to do it in Scribus.
