You're right, same here.
If I ungroup and then do Extensions/Color/Negativ i get a light gray on the smiley.
If you want a white color you can change the fill color. And the smiley has no background. So if you want to export to PNG bitmap you get the smiley with alpha.
 

Does not work.

I have opened the smiley svg, clicked on "Edit" -> "Select all in all layers",

then "Extension" -> "Color" -> "Negativ".

Color did not change.

Inkscape version: 0.48.4

Mark your objekts, use Extensions/Color/Negativ. It really change the color, cut it into stone.
