Does anyone else find the 'close' dialog box yo be confusing? It asks a question, and presents three options none of which are a direct answer to the question:
Save changes to document "x" before closing? [Close without saving] [Cancel] [Save]
Those options are effectively [No], [Cancel], [Yes] which doesn't seem a natural order for the answers either.

I personally don't find it confusing, but I'd certainly change "Save" to "Save and close", as it is the alternate option to "Close without saving". "Cancel" is fine to me, as it almost always means cancel the dialogue and go back to the app.

You read from left to right. You read the whole sentence. Then you are at the end of the sentence presented with the most likely option.
The order is a Mac and Gnome thing. If you are used to Windows it is understandable you might think it was unnatural but I trust Apple did their usability homework. It takes a little getting used to but now I wouldn't have it any other way.
Alternate button orders can now be supported but breaking the default is not an option. Alternative button orders can and should be done using the newer better infrastructure instead of breaking the defaults. (The GIMP is already working on adding support for this to their program.) The inkscape developers might not be ready to use this functionality yet depending of what version of GTK includes it. If you were willing to file a request and help out as much as you can the developers might get around to implementing alternative button order support sooner (especially if you can track down an Inkscape developer who loves KDE) but I expect it is not high on anyones priority list just yet. (Definately do please file a request though.)

Myself I have no strong opinion on this, but I just want to remark that just about any formulation of the buttons and their order can be justified and argued for. As far as I remember, our close confirmation dialog strictly adheres to HIG, so it must have some rationale behind it as well.

I see your point. However, when you think about it, there's some logic to making the buttons verbose. When the dialog appears, you can understand the resulting action of each button without having to read the dialog's message. [Yes] requires you to first know the question. On the other hand, [Save] is explicit.
