I'm not a developer, so I can't answer about the possibility of making changes. But bugs and feature requests (typically the best interface between users and developers) are managed in the bug tracker on Launchpad.

So you're welcome to search there, to find out if such a request has already been made. If it has, you can add your support to the request. Or it not, you can post a new report, and discuss with developers.

As soon as I send this, I'll try to search myself. I often don't have very good luck searching, because I don't always know the proper terminology. But I try anyway :-)

But first, I want to understand specifically what you're talking about. I'm using Windows 7, and when I click Print in Inkscape, I don't see any option for page size. That might be because I don't actually have a printer connected to this computer. Where is this option for you?

Oh wait! If I click Preferences, and then Advanced button, I get XPS Document Writer Advanced Options. And that does indeed show me Paper Size: Letter.

But I think that's part of Windows, and not part of Inkscape. Although that's just a guess on my part. Plus, my technical know-how, is more like not-know-how!

Have you noticed if you're printing from other programs, whether it automatically sets A4 or Letter? If that's the case, maybe you could print from another program, such as Adobe Reader. You could save your image as PDF, and print from there. Or you could open the SVG in a browser and print from there, *if* that provides you with the A4 option, like you want.

Maybe someone else, who understands this much more than me, could comment whether this is something Inkscape could potentially do?
