I know that it is possible to describe spot colors in Inkscape using CMYK notation. But is it possible to produce any form of output file with the colors specified in the file itself using CMYK notation? Candidate output formats would be pdf, eps, or tiff.

Yes. I actually wrote up a short blog post on this yesterday, and just need to knock out a few quick pictures.
At the moment it is use a CMYK profile, use CMS picker, import to Scribus, create PDF from there.
