Not difficult, but somewhat time consuming. The coastline could be autotraced, but the roads would be best redrawn. The railway line would take a little longer (there may be a clever way to make it with markers), and the drop shadow under the sign would have to be faked with gradients, or the blur effect.
I guess I wasn't clear. I am specifically asking about the roads in the image. The roads are outlined in black, and the outlines do not cross the places where the roads cross. Is it possible to do this in Inkscape?

Easy. Make the roads red stroke, then group them and duplicate, assign wider black stroke to the group and move it to the bottom under the red roads.
