Erm, it took me 2 minutes to draw the attached one, while you are
waiting 3 hours for a ready-to-go one. What is exactly wrong with
doing your own templates? :)
NB: proportions I used are not correct.

Will you print directly on the CD or onto a paper label?
If the latter then be warned: When you glue anything to a CD, it will
most likely become unreadable after a few months (the paper shrinks and
bends the CD until the drive can't read it anymore).
