I ended up finding the answer in the release notes. If the "Document Preferences" dialog said "Grid and Ruler units" instead of just "Ruler units" the answer would be more apparent. If that isn't acceptable it could go in a FAQ.
Grid units have no relation to the ruler. Ruler and statusbar units were simply not changeable before. Now in CVS there are default document units that apply to the ruler and statusbar. See http://inkscape.org/cgi-bin/wiki.pl?ReleaseNotes for details.
