I personally don't find it confusing, but I'd certainly change "Save" to "Save and close", as it is the alternate option to "Close without saving". "Cancel" is fine to me, as it almost always means cancel the dialogue and go back to the app.

You read from left to right. You read the whole sentence. Then you are at the end of the sentence presented with the most likely option.
The order is a Mac and Gnome thing. If you are used to Windows it is understandable you might think it was unnatural but I trust Apple did their usability homework. It takes a little getting used to but now I wouldn't have it any other way.
Alternate button orders can now be supported but breaking the default is not an option. Alternative button orders can and should be done using the newer better infrastructure instead of breaking the defaults. (The GIMP is already working on adding support for this to their program.) The inkscape developers might not be ready to use this functionality yet depending of what version of GTK includes it. If you were willing to file a request and help out as much as you can the developers might get around to implementing alternative button order support sooner (especially if you can track down an Inkscape developer who loves KDE) but I expect it is not high on anyones priority list just yet.
