OK, it shows up (very small) although I get a message saying "Image with bad reference: (path to image file)" in the status bar at the bottom of the window.

select it and use Edit->Objects to Pattern.

A "pattern2645" item (why 2645?) shows up in the pop-up menu of Fill/Pattern fill.

Now the pattern will be available in the Fill and Stroke dialog.

Yes but when I try to apply it to existing or new objects they simply appear transparent.
