It will be very difficult to do as an effect because of the "based on the pixel color below" part. An effect will have hard time figuring out this color. On the other hand, Inkscape itself already has the tool that does something very similar: Tile clones, Trace tab. It already does size and opacity, but not rotation which could be added.
