I've just found out that this might be caused by a bug in Apple's X11, not in Inkscape. On a French Usenet newsgroup (fr.comp.os.mac-os.x) one of the contributors to the French version of OpenOffice explains that "in the X11 version provided by Apple the Alt key is de-activated because the kb extension is not loaded when X11.app is launched". He also says that he has compiled a X11 version with this extension, and it works fine. He has reported this bug to Apple and it seems to appear now as "duplicate" so they wait until Apple has resolved it.
