You could save your preferred setup in a blank document and load that each time you start - just got to be careful to remember to "save as" a new name each time.
No no no. No need to do it manually. Just save as the default template and it will be loaded automatically every time you create a new file, and there's no need to "save as". The default template can be in ~/.inkscape/templates/default.svg on Linux or in [inkscape dir]/share/templates/default.svg on Windows.
I guess this should be added to the FAQ...
