You're right, same here. If I ungroup and then do Extensions/Color/Negativ i get a light gray on the smiley. If you want a white color you can change the fill color. And the smiley has no background. So if you want to export to PNG bitmap you get the smiley with alpha.

The problem appears to be related to the way in which the color information is stored in this file. The color is stored using the format fill:rgb(11.399841%,11.399841%,10.598755%). The normal Inkscape format would be fill:#1c1c19. If you create a new file from scratch, or edit the colors in Inkscape, it will work properly.
I would suggest you file a bug report at: https://bugs.launchpad.net/inkscape/

Groups made 'in InkScape' there will it work. I think the smiley is an 'Plain SVG' image.
